Blending the handier elements of ACOG forms with all of the additional information typically charted by home birth midwives, this form seeks to include the best of all worlds and show a very high level of prenatal care.
There is some color-coding discretely placed along the edges of this form, to indicate areas that are grouped together for easy assessment for growth, maternal well-being, fetal well-being, etc., making it a nice teaching tool for students.
There is also an area left open on both sides for each maternity care practice to either print their own logo and information, or apply that data on an address label. There is no wasted space on this form!
