The Prenatal Flow Sheet designed by Highland Midwife. This form has plenty of extra room for all of the visits that great midwives often have with their clients - spaces for 17 in-depth prenatal appointments, and important notes from consults/referrals.
Each appointment has a place to chart details easily without having to narrate the same things over and over in your written notes.
Lots of room on the back for advanced labs, exams, ultrasounds, dating criteria, new symptoms, important OB and health history, meds and supplements, etc. Makes it easier to track care, fill out MANA stats, and transfer information to other care providers quickly and efficiently. Pre-punched so that it flips like a page in a book for better handling in a standard top-clip chart.
Blending the handier elements of ACOG forms with all of the additional information typically charted by home birth midwives, this form seeks to include the best of all worlds and show a very high level of prenatal care.
There is some color-coding discretely placed along the edges of this form, to indicate areas that are grouped together for easy assessment for growth, maternal well-being, fetal well-being, etc., making it a nice teaching tool for students.
There is also an area left open on both sides for each maternity care practice to either print their own logo and information, or apply that data on an address label. There is no wasted space on this form!
Laser-printed on nice heavy 32-pound bright-white paper for durability and clean copying and faxing.
