The text and logo will be fit into the little empty box at the corner of each page on the charts forms.
Please send EXACTLY the text that you would like included, and email me (as an attachment) a high quality graphic of your logo; jpg, png, and gif files will all work.
Your files will be saved, so the custom fee is a one-time only charge. To order more later, simply mention in the comment box at checkout that you already have custom charts on file and are ordering more.
